How to Pay Hospital Bills in China as a Foreign Visitor
Confirm the payment route and the bill’s scope before the appointment, then keep a clear record of what was paid.
Quick answer: Confirm the payment recipient and usable method for your actual booking. A package quote, a hospital cashier transaction and a company coordination invoice can cover different items.
Payment confirmation: what to confirm
| Decision | Confirmation to request |
|---|---|
| Recipient | Hospital or company, account/channel and currency. |
| Timing | Arrival payment or a disclosed provider-required advance payment. |
| Extras | How proposed additional services are approved. |
| Documents | Payment confirmation, hospital bill and applicable tax invoice. |

Quick answer: Foreign visitors can pay for medical care in China, but they should confirm the accepted payment methods with the specific hospital or clinic. RMB cash, cards and mobile payments are possible routes; acceptance at one institution does not establish acceptance at another. Ask who receives each payment, in which currency and for which services.
For help choosing the medical route before arranging payment, see medical treatment in China for foreigners.
Confirm a primary method and a backup
The Chinese government’s payment guide for overseas visitors describes cash, bank cards and mobile payments, including linking supported international cards to Alipay or WeChat Pay. It is general visitor guidance. Ask the hospital about your card or app and the counter handling your particular appointment.
| Payment route | What to ask before the visit |
|---|---|
| International bank card | Does the relevant counter accept my card network? Can it process the expected amount? |
| Alipay or WeChat Pay | Can this hospital payment route use my overseas-card-linked account? |
| RMB cash | Where should I pay, and can that counter process this appointment’s bill? |
| Insurance direct billing | Has my exact policy and proposed visit been verified by the hospital and insurer? |
Check your bank or payment provider’s verification requirements and transaction limits before relying on an app for a substantial bill. Keep a second hospital-confirmed payment option available. A successful payment at a restaurant does not test the hospital’s own payment channel.
The Beijing government’s medical guide also lists several payment channels for international patients. Use it as a starting point for questions, then confirm the current arrangements with your selected institution.
Separate a quoted package from a hospital bill
An estimate describes planned services. A bill records charges, and a payment receipt records a transaction. They should be reconciled, but they are not interchangeable.
When reviewing an AccessChinaMed plan, ask which listed services are included, which require separate payment and who will collect each amount. A US-dollar package price is not a nationwide hospital tariff. If any part will be settled in RMB, ask how the payable amount will be confirmed; do not calculate an assumed exchange rate from a website price.
Our current cost guides set out package scope. For example, the US$199 listed diagnostics package includes spine MRI, chest CT and abdominal ultrasound, with the body region, protocol and clinical need to be confirmed. It should not be read as an unlimited imaging budget or a recommendation that every patient needs all three examinations.
Agree how additional costs will be handled
A consultation may lead to a different proposal from the one discussed before the visit. For planned, non-urgent care, ask the clinician to explain the proposed next step and the billing team to identify the associated charge before you proceed.
- Is this item already included in the agreed package?
- If not, what is the estimated additional amount and who receives it?
- Does the change affect an insurer’s earlier authorisation?
- Will another consultation, report or follow-up appointment be billed separately?
A coordinator can help communicate these questions. The clinician determines the medical recommendation, and you should understand the proposed service and payment arrangement before agreeing to non-urgent additions.
If you are asked for a deposit
Ask what the deposit reserves or covers, whether it will be credited against the final bill, and what written terms apply if the appointment changes. Request the recipient’s name, payment reference and a receipt. Keep the deposit record so you can check that it is accounted for at settlement.
Confirm any cancellation or refund process before paying, including the person or department handling it. Do not infer a refund deadline from a different hospital’s website. For a card payment, distinguish an amount actually charged from any temporary authorisation your card provider shows.
Keep documents that explain the transaction
Ask the billing team what invoice or official medical billing document it issues, sometimes referred to as a fapiao, and whether an itemized statement is available. Keep proof of payment alongside it. An app transaction screen alone may not explain which medical services were provided.
Check the patient name, visit date, amount, currency and any separate bill references. If your insurer or employer needs a specific format, ask before leaving; our insurance guide covers the questions to put to the recipient.
For medical documents in English, see what to request from the hospital. Payment evidence and clinical reports serve different purposes, so retain both when required.
A real example: why checking the line items matters
In the records for our Shenzhen visit case, one hospital receipt records CNY549: CNY299 for medicines, CNY238 for traditional Chinese medicine treatment and CNY12 for another treatment charge. These line items add up to the receipt total. They do not establish the complete cost of the trip or a standard price for another patient.
The same case has a separate historical team price record in US dollars. We do not add it to the CNY receipt or present it as a verified paid total, because the records describe different scopes. This distinction is useful whenever an itinerary includes a package plus separately billed hospital services.
Before leaving the cashier
Confirm which charges are settled, whether a deposit balance remains and how any outstanding invoice will be delivered. Keep the billing contact and reference rather than relying on a general hospital switchboard later. If payment appears to have failed, ask the cashier to check the transaction before attempting the same charge again.
AccessChinaMed projects include free English appointment accompaniment through December 31, 2026. We can help clarify the visit’s practical payment questions; hospital charges and any separately agreed services should remain visible in your individual plan.
